List every AcroForm field in a PDF with its dotted name, type, values, options, flags, tooltip, and position, and flag unnamed or tooltip-less fields.

The report walks the AcroForm field tree: fully qualified dotted names built from parent chains, types derived from the FT entry and flag bits (button flags 16 and 17 split checkbox, radio group, and push button; choice flag 18 splits combo box from list box), stored and default values, choice options with their export values, tooltips, and each widget's page and rectangle in points before any page rotation. JavaScript attached to fields is counted and never run. XFA forms are detected, not parsed. Encrypted documents keep their structure listed but their names, values, and tooltips stay blank because strings cannot be decrypted here, and drawn boxes that are page graphics rather than real fields never appear. Nothing is uploaded.
